What is Evidence of Insurability (EOI)?
Some insurance plans require you to show you are in good health. This is called Evidence of Insurability, or EOI, and is most common with life and disability plans.
During the EOI process, you will typically need to complete a health questionnaire that provides details about your personal medical history, and perhaps even the medical history of your spouse. You may also be required to get a physical examination with blood work. An EOI is typically required when you apply for an amount that is higher than what the insurance company calls a “guaranteed issue” amount, or, you apply for coverage outside of your designated new hire enrollment period. The insurance company will review the information you submit and notify you of the results.
